Medical Facilities
Dialysis
A residence for individuals who are at least eighteen years of age, have behavioral health issues and assistance with activities of daily living.
Home Health
An assisted living facility that provides resident rooms or residential units to eleven or more residents.
Hospice
An assisted living facility that provides resident rooms to ten or fewer residents.
Outpatient Treatment Center
A residence for individuals who are at least eighteen years of age, have behavioral health issues and assistance with activities of daily living.

Rehab
A residential setting that provides care for at least one and no more than four adults who are participants in the Arizona long-term care system. The manager resides with the residents and integrates the residents who are receiving adult foster care into that person’s family.
Long Term Care Providers
INTERMEDIATE CARE FACILITY FOR INTELLECTUALLY DISABLED
The primary purpose of the institution is to provide health or rehabilitative services for the intellectually disabled individuals or persons with related conditions and receives active treatment.
SKILLED NURSING FACILITY
A skilled nursing home is defined as an institution (or a distinct part of an institution) which is primarily engaged in providing skilled nursing care and related services for residents who require medical or nursing care, or rehabilitation services for the rehabilitation of injured, disabled, or sick persons, and is not primarily for the care and treatment of mental disease.
Residential Services Providers

Adult Behavioral Health Home
A health care institution that provides
continuous treatment to an individual experiencing behavioral health issues.
Group Home
A community residential setting for not more than six persons with developmental disabilities that is operated by a service provider under contract with the department and that provides room and board and daily habilitation and other assessed medically necessary services and supports to meet the needs of each person.
